Job Description Job Summary Will be a Licensed Practical Nurse
performing duties as a patient navigator guiding incoming calls and
facilitating entrance of new patients into the program. Provides
patient education, promotes an understanding of the condition, and
provides ongoing continuity of care for patients. The Nurse
Navigator, functioning in the spirit of teamwork, is accountable
for the delivery of care utilizing skills learned in both the
clinic and didactic arena. Assists other team members on the unit.
The nurse navigator role provides clinical support to the provider
and patients to assist in the diagnosis and treatment of
conditions. Job Duties Hours/Schedule: Day/Evening position
(40hrs/week) - Hours will fall between 7:00 a.m. and 11:30 p.m. -
Shifts may be 12, 8, 6, or 4 hours in length. - Schedules are
created based on patient needs. During orientation, LPNs will work
Monday–Friday, typically between 8:00 a.m. and 5:00 p.m. After
orientation, LPNs will continue Monday–Friday, 8:00 a.m.–5:00 p.m.,
until enough staff are hired and trained to support expanded
coverage. Prior to the transition into expanded hours or full 24/7
coverage, leadership will communicate scheduling changes in
advance. Weekend Requirements - All positions require working every
other weekend. - Weekend shifts are 12 hours in length. - Weekend
assignments are based on department needs and scheduled equitably.
- If staffing exceeds core needs, leadership may pull staff off
using a rotating seniority system to ensure fairness. Holiday
Requirements - All LPNs work at least one major and one minor
holiday per year. - Holiday shifts are 12 hours. - If staffing
exceeds core needs, leadership may remove a nurse from the holiday
schedule using the same rotating seniority-based fairness process
Travel: Less than 25%- Travel may be required for
orientation/training, meetings, or other business needs.
Experience: A minimum of 2 years previous LPN work experience is
required. Clinic and Pediatrics experience is preferred. Proficient
computer skills. Proficient with Microsoft products such as Word,
Excel, PowerPoint, SharePoint, Windows, & TEAMs. Job Duties: -
Utilizes clinical knowledge to determine actions necessary to
facilitate patient introduction to the program. - Performs and
documents evaluation of patients needs primarily by telephone or
review of documentation. - Effectively collaborates with other
healthcare providers in coordinating and implementing the patients
plan of care, to include evaluation, treatment, and surveillance. -
Effectively communicates with patient via mail, telephone, and in
person regarding information pertinent to their care. - Assures
pertinent patient information is received and available for
provider interaction with patient. - Coordinates patient needs for
the program. - Serves as the point of contact for providers. -
Coordinates patient activities, such as support groups, educational
series, as appropriate. - Provides limited patient and family
education based on individual needs either in person or by
telephone. - Coordinates patient activities through available
support services, as appropriate. - Maintains systems and processes
sufficient to assure timely follow-up and surveillance of patients
for both Programs. - Serves as a liaison with Scheduling Services
to ensure appropriate scheduling of patients. - Develops and
implements patient screening tools and efficient clinic procedures.
- Assists in the development and implementation of efficient
patient navigational procedures. - Investigates patient concerns
including appointment access, communication, and billing. -
Maintains clinical supply inventory. - Responsible for conducting
medical record reviews to report findings to physician specialist.
- Provides nursing care and assists physician and procedures. -
Routinely inspects and observes condition and operation of
equipment to ensure proper operation and identify the need for
preventive and corrective maintenance per guideline. - Performs
ongoing maintenance of exam and treatment rooms. Work is typically
performed in a clinical environment. Accountable for satisfying all
