Job Description:
MedStar Health, a leading not-for-profit healthcare organization
located in the D.C.-Maryland-Virginia region, is seeking a
dedicated RN Inpatient Case Manager to join our patient-first team.
In this role, you will be responsible for overseeing the continuum
of care for inpatients, aiming to optimize health outcomes and
patient satisfaction while ensuring cost-effective care. This
position requires strong interdisciplinary collaboration and a keen
ability to manage multiple patient cases efficiently. Assess
patient needs and develop detailed care plans in collaboration with
physicians and other healthcare providers. Monitor patient progress
and adjust care plans as necessary. Facilitate patient transitions
between care settings, ensuring continuity and the optimization of
resources. Educate patients and their families on treatment plans
and health management strategies. Collaborate with insurance
providers to ensure coverage of healthcare services. Maintain
comprehensive records of patient care, treatments, and progress.
Strong analytical and problem-solving skills. Effective
communication and interpersonal skills. Proficiency in electronic
health record (EHR) systems. Ability to work collaboratively across
various healthcare disciplines. Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Current registered nurse (RN)
license in the state of practice. Certification in Case Management
(CCM) preferred.
