Summary The Licensed Practical Nurse (LPN) provides skilled nursing
care to patients in the home setting to support optimal health and
independence. The LPN delivers care as directed by the RN and
physician, focusing on routine visits and treatments for patients
with acute or chronic conditions. The LPN ensures continuity of
care through effective communication and thorough documentation and
collaborates with the interdisciplinary team to meet patient needs.
Reports to the Home Health Manager. Duties and Responsibilities
Deliver nursing care according to organizational policies,
procedures, and the LPN scope of practice. Document all care
provided through clinical and progress notes, emphasizing outcomes
of interventions. Assist the RN and/or physician in performing
specialized patient care duties. Carry out tasks outlined in the
established plan of care under RN supervision. Prepare necessary
materials and equipment, maintaining aseptic technique when
required. Administer treatments including wound care, catheter
care, and medication administration as prescribed. Educate patients
on appropriate self-care practices and encourage independence.
Assess and document patient and family responses to care provided.
Communicate relevant patient information to appropriate members of
the care team. Participate in weekend, holiday, and on-call
rotations as scheduled. Perform other duties as assigned.
Requirements: Minimum of one (1) year nursing experience preferred,
ideally in Home Health, Hospice, or Skilled Nursing. Current,
unencumbered state license as a Licensed Practical Nurse (LPN).
Current Basic Life Support (BLS) CPR certification. Ability to
read, write, and speak English fluently. Strong verbal and written
communication skills. Capable of working independently, meeting
deadlines with limited supervision. Ability to perform CPR and
manage airway obstruction emergencies. Physically able to climb at
least two flights of stairs and stand or walk for at least four (4)
continuous hours. Valid state driver’s license. Satisfactory motor
vehicle driving record per agency and insurer standards. Proof of
valid automobile insurance. Must complete 12 hours of continued
education, annually.
