Job Description Job Description Position Overview: The Case Manager/Delegating Nurse Consultant provides expert nursing services, focusing on training, supervision, and assessment in a developmental disabilities setting. They assist in medical care planning, medication administration oversight, and ensure adherence to regulatory standards. This role is consultative and does not involve direct patient care but ensures proper medical care and compliance for individuals with developmental disabilities. Key Responsibilities: Health and Nursing Assessments: Conduct assessments for each new individual admitted to the CLA within 30 days. Perform monthly, quarterly, and annual nursing reviews for each individual. Develop individualized nursing care plans for those requiring medical involvement or nursing delegation. Interdisciplinary Team Participation: Attend post-admission conferences and annual reviews as a member of the interdisciplinary team. Participate in case discussions with physicians, staff, and individuals on health-related issues. Monitoring and Compliance: Review medical and dental records regularly to ensure appropriate care and follow-up. Monitor physician and dentist orders monthly and ensure correct implementation. Ensure adherence to medical/medication protocols and policies, addressing incidents and ensuring appropriate medical attention. Medication Administration Oversight: Review Medication Administration Records (MAR) and physician/dentist orders monthly. Conduct periodic audits of medication administration by Certified Medication Technicians (CMTs). Monitor compliance with DDA regulations on medication handling, reporting errors, and documenting administration procedures. Documentation: Consult staff on DDA documentation standards, ensuring proper recording of treatments, medications, and medical history. Ensure seizure documentation is maintained per DDA regulations. Review individual records for proper documentation of behavior-modifying medications and physician rationale. Training: Provide training to new employees and supervise initial worksite medication administration by CMTs. Conduct annual training/testing for CMTs, and provide retraining as needed. Offer on-site training on changes in medical conditions and care plans, and other relevant topics. Additional Responsibilities: Coordinate with physicians, staff, and hospital case managers for discharge planning. Conduct pre- and post-hospital re-admission assessments. Ensure staff adherence to medication administration protocols and assist with corrective actions if necessary. Qualifications: Registered Nurse (RN) licensed in Maryland. Knowledge of Maryland Nurse Practice Act and DDA regulations. Experience in case management, nursing delegation, and medication administration oversight. Strong communication, training, and organizational skills. Working Conditions: Collaborative work with health teams, direct care staff, and physicians. Regular reviews, assessments, and consultations in a community-based environment.
