Sign-On Bonus Available! The Nurse Practitioner is responsible for
conducting thorough in-home health evaluations for adult and
elderly patients, with an emphasis on enhancing care quality and
pinpointing health risks. This role requires collaboration with
physicians, caregivers, and case managers, while offering the
flexibility to work in various environments, including private
residences and skilled nursing facilities. The position comes with

the Role During visits that may last up to one hour, our providers
assess a member’s current health status, medical history,
medication adherence, social circumstances, and other potential
risks. This approach provides invaluable insights into a member’s
overall health that are often missed during standard office visits.
The Comprehensive Health Assessment (CHA) enhances care quality and
allows us to address multiple care gaps in a single visit. Our
providers: Perform assessments for adults and geriatrics, including
medical history, diagnosis, treatment, health education, physician
referrals, case management referrals, follow-ups, and thorough
documentation in accordance with our guidelines and protocols.
Administer and order necessary point-of-care and lab tests.
Collaborate with physicians, case managers, social workers, family
members, key caregivers, and other medical personnel as
appropriate. Work alongside Primary Care Physicians (PCPs) to
educate patients and ensure necessary follow-ups. Provide services
at the member’s residence, whether in their home or in Skilled
Nursing Facilities. Benefits of Working with Us Flexibility –
Assessments are scheduled according to YOUR availability. We
conduct evaluations from Sunday to Saturday, between 8 am and 7 pm.
You can choose to work during the day, evening, or weekend based on
your preferences. Competitive Pay – Your income is based on the
number of assessments completed. You can increase your earnings by
being more available and traveling to see members outside your home
area. Those averaging 20 to 100 visits per month can earn between
$1,800 and $11,500 , depending on the per assessment rate of $90 to
$115 based on your state of residence. Benefits – Includes mileage
reimbursement, a 401(k) plan, employee referral bonuses, and

your development! Along with comprehensive onboarding training, you
will be assigned a preceptor, receive personalized feedback, and
have free access to continuing education opportunities. Additional
Licensure – If business needs arise, we will assist you in
obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Full-Time Opportunities – Our PRN providers
have the option to transition to full-time roles supporting their
home location or a specific state or region, along with a full-time
sign-on bonus. Qualifications Skills & Experience That Shine A
Master’s Degree or equivalent experience, along with successful
completion of NP licensure. Current NP licensure in the state of
practice, including prescription authority or the ability to obtain
it as necessary. AANP, AACN, or ANCC board certification in Adult,
Adult Gerontology Primary Care, Adult Gerontology Acute Care,
Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or
CPR certification. Preferred one year of NP experience; new
graduates are encouraged to apply! A valid state driver’s license
and proof of adequate automobile insurance for your state of
residence. Strong computer skills and familiarity with
health/medical record software. Excellent verbal and written
